PROJECT OBJECTIVE 
​
The goal is to build capacity for collaborative transition service delivery within a Utah school district and improve outcomes for transition-aged students with disabilities. We are creating UNITE, Unified Networks in Transition Excellence, an innovative co-mentoring program to promote collaborative transition service delivery.

PROJECT PROCESS
 
We are conducting a co-design process with general educators, school counselors, and special educators from each of the district's schools and the young adult program, along with two district-level administrators. Together, we examine transition service delivery at the school- and district-levels and design system support to build capacity for collaboration. â€‹â€‹â€‹
